  1. Report from Board of Examining Chaplains regarding the General Ordination Examinations; and report on any remedial work.
  2. Endorsement letter from the Member of the Clergy in Charge of the Congregation
  3. Endorsement letter from the Vestry/Bishop’s Committee. (See Appendix H.)
  4. Medical and Psychological Examinations.
  5. Certificate from the theological school showing the Candidate’s scholastic record and evaluation with recommendation as to the Candidate’s other personal qualifications for Holy Orders and recommendation regarding Ordination.
